Excess Student Accident Insurance Summary

2017-2018 SCHOOL TERM

The School District of Hillsborough County student athletes are required to pay a Sports Insurance Activity Fee before they will be allowed to condition, practice or compete in Hillsborough County District sanctioned/FHSAA interscholastic sports that are outlined below. The Sports Activity Insurance Fee provides basic accident insurance for the student athletes while participating in District sanctioned/FHSAA try-outs, practices and games. Enrollment Forms are available from this website, or the school Coach or Athletic Director’s Office. These plans does not provide coverage during any summer practices, drills, open facilities, summer leagues or games. This coverage becomes effective on the first District/FHSAA sanctioned practice date or on the date paid, at 11:59 PM, whichever is the later date.

Retain the downloaded application for your records. If you have questions contact School Insurance Of Florida. We cannot accept enrollments or payment of the Sport Activity Fee or bind coverage by telephone or fax. You may also fill out an enrollment application, attach a money order payable to School Insurance Of Florida and return the completed enrollment form and payment to your Coach or Athletic Director’s Office. The basic insurance plan will not pay for 100% of all medical charges incurred and is not designed to replace family insurance or primary insurance plans.  This will supplement your primary insurance plan.

GROUP A DISTRICT SANCTIONED/FHSAA TACKLE FOOTBALL AND LACROSSE $60.00 -Hillsborough County School District Sponsored Tackle Football and Lacrosse, as sanctioned by the FHSAA. The 2017-2018 tackle football and lacrosse coverages expire after the last FHSAA/District sanctioned practice or game. Also provides coverage for off-season weightlifting* on school premises while under the direct supervision of a school coach.  This plan will expire on the last day of school. Also provides coverage for Groups B and C.  

GROUP B HIGH SCHOOL DISTRICT SANCTIONED/ FHSAA INTERSCHOLASTIC SPORTS $40.00 - Provides basic accident insurance during SDHC Soccer, Volleyball, Baseball, Softball, Wrestling, and Basketball practices and games during the 2017-2018 regular school term, beginning in August, as sanctioned by the FHSAA. Also provides coverage for conditioning* on school premises while under the direct supervision of a school coach. Includes coverage for the sports listed in Group C.

GROUP C HIGH SCHOOL DISTRICT SANCTIONED/FHSAA INTERSCHOLASTIC SPORTS $30.00 - Provides coverage only for SDHC Cheerleading, Golf, Cross Country, Track, Swimming, Girls Flag Football, Tennis, Marching Band, Auxiliary Units, ROTC and Team Trainers/Managers during school sponsored practices and competition during the regular 2017-2018 school term, beginning in August, as sanctioned by the FHSAA. Also provides coverage for off-season weightlifting* while on school premises and while under the direct supervision of a school coach.  Policy provisions and exclusions apply.

MIDDLE SCHOOL DISTRICT SANCTIONED SPORTS $25.00 - Middle School Track and Field, Soccer, Volleyball, Basketball and Boys/Girls Flag Football sponsored practices and competition during the regular 2017-2018 school term, beginning in August. Team Trainers/Managers are included in this option.

JROTC Drill Participants $30.00 - Provides coverage for JROTC after school district sanctioned activities that are exclusively scheduled, organized and sponsored by the SDHC and supervised by a JROTC designated instructor during the regular school term. Coverage is also provided for the sports listed above in Group C.

SDHC BAND MEMBERS AND AUXILIARY UNITS $30 - Provides coverage for SDHC sanctioned marching band activities that are exclusively scheduled, organized and sponsored by the SDHC and supervised by a SDHC district instructor during the regular school term. Coverage is also provided for the sports listed above in Group C and or JROTC.

The 2017-2018 enrollment application is available to download for your records. You may enroll by filling out the enrollment application and returning the completed application to your coach along with your money order or Purchase Online Now and enroll with a visa or mastercard. If you elect to pay with a money order make your payment out to School Insurance of Florida. Retain the top part of the enrollment application for your records. The coverage is effective on the date paid at 11:59 PM. Policy provisions and exclusions apply. COVERAGE EFFECTIVE AND TERMINATION DATES

If a student has paid the required Sports Fee before the first day of a school sponsored, school supervised FHSAA athletic practice, the coverage goes into effect on the first day of sanctioned FHSAA practice.  If the insurance fee is paid after the first day of practice, coverage starts the day after the enrollment and payment of the fee. Coverage cannot be backdated.  Coverage under the school sports insurance plan is effective only while Hillsborough County School students are participating in interscholastic sports practices and competition that are exclusively school sponsored, school scheduled, school supervised and school funded during the regular school term, and as sanctioned by the FHSAA. Injuries during other non-sports related activities and regular classes are not covered. This plan will provide coverage for off-season cardio conditioning and weightlifting while under the direct supervision of a SDHC coach. Conditioning as "Weight training is the use of free weights and stationary apparatus. Cardiovascular conditioning is distance and interval training. Plyometrics is the use of pre-set conditioning programs. Conditioning IS NOT teaching sport specific skills and drills, and DOES NOT involve the use of sport specific equipment (i.e. starting blocks, hurdles, rebounders, ball machines, bats, balls, rackets, etc.) and is not covered under this policy. 

All coverage under the school sports insurance policy terminates after the last FHSAA interscholastic sports game scheduled during the regular school term or on the last day of school whichever is first.  Coverage does NOT continue during the summer months for any practices, games, drills, open gyms, camps or leagues.
